East End Behavioral Health Hospital is Permanently Closed
As of January 31, 2024, East End Behavioral Health Hospital is permanently closed.
To Request Medical Record Copies:
Medical records for past patients will be retained in accordance with the State of Pennsylvania medical information preservation guidelines. After the date of closure, the responsibility for maintenance of the medical records will be transferred to LifeCare 2.0 , which will act as the custodian of those records. Former patients or their authorized representative can request records created at East End Behavioral Health through one of the following methods:

Call 214-473-3220 or 214-473-3223 to request the Authorization paperwork.

Mail a written request and photo identification to 6800 Preston Rd, Plano, TX 75024, or

Fax a written request and photo identification to 214-473-3222.

Email authorization request and photo identification to EEB.MedRec@lifecare-health.com.
All requests for medical records must be in writing and copy fees will not be applied for requests for six months after the date of closure. Six months after that date, medical record copy fees will apply per Pennsylvania law.
